Bottom line
If you have full Maryland Medicaid, your first step is usually the Healthy Smiles program, which covers many dental services for eligible Medicaid members with no premium, deductible, copay, or yearly dollar cap for covered care.
If you do not have Medicaid, start with a community health center, county dental clinic, dental school clinic, or the state oral health guide and ask for sliding-fee care. If you are in pain today, call early because same-day dental slots are limited.
This guide focuses on real help, not fake “dental grants.” For a national overview, see our dental help guide too.
Urgent dental pain or swelling
Call 911 or go to an emergency room if you have trouble breathing, trouble swallowing, swelling that is spreading, fever with facial swelling, heavy bleeding, or a serious injury to your mouth or jaw. An ER may not fix the tooth, but it can treat dangerous infection or injury.
For urgent tooth pain, swelling, a broken tooth, or a likely extraction, the UMSOD urgent clinic in Baltimore is a major walk-in option. It has limited weekday morning slots, participates with Maryland Medical Assistance and Maryland Healthy Smiles, and tells patients to bring ID, insurance information, medication lists, and payment method.
You can also dial 211 Maryland and ask for “urgent dental care near my ZIP code today or tomorrow.” Say whether you have Medicaid, no insurance, pregnancy coverage, or a child in pain.
Where to start
If you have Medicaid
Use the member portal or call Maryland Healthy Smiles at 855-934-9812 to find a dentist. Ask the office if it is taking new Healthy Smiles patients before you go.
If your child needs care
Children may qualify for Medicaid or MCHP even when a parent does not. The MCHP page explains that MCHP includes dental care for eligible children.
If you are pregnant
Ask Maryland Health Connection which pregnancy or postpartum coverage applies. Maryland says pregnant Medicaid coverage can include dental care, and some coverage groups have different postpartum rules.
If you are uninsured
Call a clinic before paying private rates. The HRSA locator can help you find federally funded health centers that may offer dental care on a sliding fee.

Quick reference table
| Situation | Best first step | What to ask |
|---|---|---|
| Adult with full Medicaid | Healthy Smiles program | “Can you help me find a dentist taking new adult Medicaid patients?” |
| Child or teen | MCHP page | “Can my child get dental coverage even if I do not qualify?” |
| Pregnant or postpartum | pregnancy Medicaid | “Which pregnancy or postpartum dental coverage applies to me?” |
| Uninsured adult | oral health guide | “Do you have sliding-fee dental care or an emergency appointment?” |
| Tooth pain today | UMSOD urgent clinic | “What time should I arrive, and what should I bring?” |
Maryland Healthy Smiles dental coverage
Maryland Healthy Smiles is the dental program for eligible Maryland Medicaid members. The state says full Medicaid members are eligible, and the covered services include regular checkups, cleanings, X-rays, fillings, extractions, root canals, crowns, anesthesia, fluoride treatments, and certain child-only services such as sealants and medically necessary orthodontics.
Adults can get denture adjustments, but Maryland says dentures for adults age 21 and older are not covered. Cosmetic and experimental care is also not covered. The MDAC adult dental summary is useful, but the state program page should be your final check before major treatment.
Tip before you book
Do not rely only on an online provider list. Call the dental office and ask, “Are you taking new Maryland Healthy Smiles patients this month?” Office participation can change.
If you need help with Medicaid basics, our Medicaid guide explains common starting points for parents and children.
How to apply for Medicaid, MCHP, or a dental plan
You can apply for Medicaid or MCHP at any time. The state Medicaid application page lists online, phone, app, local health department, and local Department of Social Services options.
Maryland Health Connection is also where many families shop for private health and dental plans. The Maryland dental plans page says Medicaid and MCHP dental care is free for enrolled members, while private dental plans have their own premiums, networks, and adult cost rules.
Pregnancy coverage needs extra care. Maryland says eligible pregnant people can apply during pregnancy and Medicaid coverage can extend after pregnancy for comprehensive medical, dental, and postpartum care. The Healthy Babies page also explains a pathway for some pregnant noncitizens and says that dental care is included. Because postpartum rules can differ by coverage group, ask a Maryland Health Connection assister which rule applies to you.
For more help during pregnancy or after birth, our postpartum guide can help you sort the next calls.
Low-cost dental care if you do not have Medicaid
If you are uninsured or your plan will not cover the work you need, ask about sliding-fee clinics, dental school care, county programs, and charity dental programs. The federal HHS dental options page lists community health centers, dental schools, Medicaid, CHIP, VA dental care, and clinical trials as common low-cost paths.
| Option | Best for | Reality check |
|---|---|---|
| Community health centers | Cleanings, fillings, extractions, and routine care | Ask for the sliding-fee application and bring income proof. |
| Dental school clinics | Urgent care and lower-cost supervised care | Student clinics can take longer than private offices. |
| County dental clinics | Local basic or urgent dental care | Eligibility may depend on county, income, insurance, and age. |
| Donated dental care | Adults with disability or serious medical need | Expect screening and possible waits; it is not same-day care. |
The Maryland Foundation Dentistry coordinates donated dental care for adults with disabilities who lack insurance or money for treatment. Call before applying so you understand the eligibility rules and wait times.
Where to call by area
The state oral health guide is organized by county, and it is often the fastest way to find local public dental programs. Here are practical starting points by region.
| Area | Starting point | What to ask |
|---|---|---|
| Baltimore City | Baltimore City clinics | Ask if you qualify for preventive or urgent care at Druid or Eastern Dental. |
| Baltimore region | Dental school and FQHCs | Ask about urgent openings, Medicaid, and sliding-fee care. |
| Eastern Shore | Choptank locations | Ask which dental site has adult appointments and emergency openings. |
| Lower Shore | Chesapeake dental | Ask about Princess Anne or Salisbury dental appointments. |
| Any county | 211 resource search | Search by ZIP code for dental care, transportation, and medical bill help. |
If dental care is one part of a bigger crisis, our community support, food help, child care help, and housing help pages can help you sort the next step.
What to bring or have ready
Ask each clinic what it needs before you travel. Most dental offices and public programs may ask for some of the items below.
- Photo ID, if you have one.
- Maryland Medicaid card, Healthy Smiles card, private insurance card, or MCHP information.
- Proof of address, such as mail, lease, benefit letter, or school record.
- Proof of income for sliding-fee programs, such as pay stubs, benefit letters, or a written statement if income is irregular.
- List of medications, allergies, medical conditions, and recent blood work if relevant.
- Pregnancy information if you are pregnant, especially if urgent dental treatment may involve medicine or X-rays.
- A denial letter, bill, or treatment plan if you are asking for an appeal, payment plan, or complaint help.
Do not skip the call
Sliding-fee clinics may use different rules. Ask what papers they need and whether they can still see you if one paper is missing.
Rides to dental appointments
If you have Medicaid and no other way to get to a covered medical appointment, ask about Non-Emergency Medical Transportation. The state NEMT page says Maryland Medicaid helps recipients access medically necessary Medicaid-covered care through NEMT services.
Transportation is not for emergencies. Some county programs screen applicants and may need advance notice. If you need a dental ride, call your local health department or the number on your Medicaid plan materials as soon as the dental office gives you an appointment.
For more Maryland options, our ride help guide lists other starting points.
Common mistakes to avoid
- Assuming every dentist takes Medicaid. Always call first and ask if they are accepting new Healthy Smiles patients.
- Waiting on a tooth infection. Swelling, fever, or trouble swallowing can become dangerous. Get medical help right away.
- Paying out of pocket for covered Medicaid care. If you have full Medicaid, ask Healthy Smiles before paying for a covered service.
- Assuming adult dentures are covered. Maryland says adult dentures are not covered, though adult denture adjustments may be covered.
- Missing appeal deadlines. Save denial letters, bills, and treatment plans. Ask for help quickly.
If you are denied, delayed, billed, or ignored
First, ask the dental office for the reason in writing. If Medicaid is involved, call Healthy Smiles and ask whether the service is covered, whether prior authorization is needed, and whether the dentist is in network.
If a private dental or health plan denied care your provider says is medically needed, the MIA appeals page explains Maryland appeal and grievance steps. If the problem is a medical bill, insurance dispute, or coverage dispute, the Attorney General’s HEAU complaints page says its mediation services are free.
If your legal issue is bigger than a dental bill, our legal help guide can help you find a starting point.
Backup options when appointments are full
- Call Healthy Smiles and ask the agent to help search by ZIP code and nearby counties.
- Call early in the morning and ask FQHC dental clinics about cancellations.
- Ask whether the clinic can do a limited exam for the painful tooth first.
- Check the state oral health guide for county programs and special-needs dental programs.
- Ask 211 for one-day clinics, church-based help, or local medical bill help.
- Ask dental offices whether they offer payment plans only after you check Medicaid, sliding-fee, and clinic options.
